We incise and drain abscesses and remove pus. Septic arthritis is surely an abscess in a joint and pus must be drained. The exception is TB where I feel antibiotics alone can be effective if given early. Too many joints are destroyed because pus is not drained in time. This gives lifelong morbidity.
